BY CLICKING THE "SUBMIT" BUTTON ABOVE, I hereby acknowledge that Additional Insured status is not automatically included under my policy and that an additional premium may be charged. I also agree to notify Aspen Agency, Inc. if either (a) the project is completed and the Additional Insured request is no longer needed or (B) the Additional Insured request needs to be extended into a subsequent policy period. I further acknowledge that Aspen Agency Inc. has advised that a written contract between my business and Additional Insured is recommended.
